Adding a Defect
It is important that when an individual uses a piece of equipment that it works, this means recording maintenance for that piece of equipment as well as logging any defects that might occur, once a defect is logged it’s important to then track it against its impact to the business and record its rectification. This can be all achieved within Centrik, additional configuration can also alert an individual in an organisation when a defect has been raised.
Adding a defect when part of a check
1. Navigate to the piece of specific equipment, either through the equipment landing page, or by using the equipment search function
2. Click on the ‘Add Check’ button
3. Carry out the list of checks, selecting from the dropdowns available. If an item fails the checklist due to a defect, select this from the available list and add a comment if necessary
4. Click on the ‘Save’ button
5. Within the Defects section click on the ‘Add’ button
6. In the Defect section complete a title of the defect, and select a priority, and a severity of this defect to the business from the dropdowns available.
The next section relates to the individual responsible for rectifying the defect and is titled Responsible Person. It is within this section that the individual responsible for fixing this defect can be inputted. There is a selectable option of this individual being an External Contact, in essence they don’t have access to your Centrik system, selecting this as YES will create areas for an External Contact Name and External Contact Email to be added. If the individual responsible is an internal contact, then typing in their name in the available field will allow you to select from the list.

You can add any attachments by dragging and dropping these into the attachments area.
7. Click on the ‘Save’ button
Now that a defect has been raised you are able to view it within the class tab of that equipment under a section titled Defects Open.
Clicking on the number icon will take you to the item of defective equipment. Alternatively, you can click on the Defects tab.
If the responsible person is an internal contact their name will be displayed, if they are an external contact, it will not be shown.
There is a pop out box also displayed, clicking on this will show the details of the defect in a more granular way.
8. Decide if this defect will Fail the check being carried out, or if the check is OK to sign-off but requires repair at some stage.
This can be actioned within the Result area, click on the ‘Edit’ button and choose from the dropdowns in the Result field
9. Click on the ‘Save’ button
Navigating back to this specific piece of equipment will highlight any open defects, there is also the ability on this page to add a defect outside of a check by clicking on the ‘Add’ button and following the same process.
You can view all defects by clicking on the ‘View All Defects’ button.
Once again, a pop box option is available to use if required, this allows you to scroll all the defects one by one by using the ‘Next’ and ‘Previous’ buttons.
