Audits are created as checklists within Centrik, several distinct types of checklists can be configured by Centrik for your system.
Checklist Type – Standard checklist type
Self-Audit – The ability to create organisational check lists
Third Party – The ability to create and utilise audit over email and create different organisation types within Centrik
Survey – The ability to create Surveys, these can be used internally or externally and allow for the creation of bespoke survey type audits
IOSA Style – The ability to create IOSA style audits
Regulator – Allows authorities to plan inspections, conduct audits and have oversight of organisational compliance
With exception of Surveys all checklists can be configured to have ‘Findings’ enabled.
The only types of audit that can have a schedule attached to them are Checklist audits and IOSA style audits. This can be set at either 12 or 24 months.
Checklists for Internal, External and Self-audits are all created in the same way. Third party audits, Surveys and Regulator style audits have checklists within each type but there is a different process in how these are created.
+ New Checklists – provides the ability to create a checklist, link them to an audit and create a schedule for them (depending on the audit type, and subsystem presets)
1. Click on the ‘+ New Checklist’ button from the Checklists schedule landing page
2. Select the audit type from the dropdown menu
You are directed to a screen where you can select the audit style, the audit answers, if the audit can be edited, and the audit schedule.
The audit phases or stages of the audit can be seen on the right hand-side of the screen.
3. Enter in the details of the audit within the Checklist Detail section, this includes the checklist Number, Checklist Name, Checklist Style, Possible Answers, Audit Editability, and a description (if required)
Checklist Number – This number is auto populated to be the next number increment; it defines where in the list of checklists this will be displayed within that subsystem; this can be overtyped though if required.
Checklist Name – The name of our checklist (what you see on the landing page)
Checklist Style – the style of audit you wish to use, each style offers different options.
Classic – The ability to create checklists sections and create your own checklist / audit questions
Classic with evidence – The same as classic style but with an additional area to capture evidence
Regulation based – The ability to create a checklist audit based on a regulation, when linked to Regulations this will create your audit question directly from the Regulation
Regulation based with evidence – The same as regulation based but an additional area to capture evidence
Regulation based with Auditor Actions (IOSA style) – The ability to create a checklist that uses the IOSA style of questions and answers
Possible Answers – the ability to select how you score each item against conformance. If selecting Regulator based with auditor actions (IOSA style) as your checklist style the possible answers will default to IOSA Audit Result
Audit Editability – This setting has three options available; Checklist is extendable (additional items may be added, but items copied from Checklist are fixed, checklist is fixed (it cannot be edited), Checklist is editable (items can be added, removed, amended)
Description – optional but the ability to put a description regarding this checklist
4. Next, we move onto the schedule section
Within the schedule section you can enter in the last completed on date (optional) or start to plot your checklist schedule by ticking the checkboxes for the displayed months.
5. Click on the ‘Save’ button
After saving additional sections appear within this checklist, this is where we can start to add our checks.
Comments – Any changes, amendments to this audit are captured within the comments field. Comments can also be added by using the ‘+ Add comment’ button.
Comments are date and time stamped alongside the comment creators’ details.
Form Sections - The ability to add in sections to your audit, these can be enabled by Centrik
Sections and Contents - This is the area that will host your audit questions broken up into sections
1. Clicking on the ‘+Add New Section’ button will allow you to create an audit section. It will need a Section Number (numerical or alpha numerical) and a section name. The description field is optional.
The audit phases can be seen on the right hand-side of the screen.
2. Adding items to the section is achieved by using the ’Add Heading’ button or the ‘Add Item’ button. A heading cannot hold a reference within it, but an item can.
3. Enter in a heading title (if using) and any checklist items required for this section
4. Once all headings (optional) and items have been completed you are able to click the ‘Renumber’ button, this will place a numerical value for each heading or item
After clicking the ‘Renumber’ button, if using headings the number will show as a whole number for the heading followed by a decimal for each item within that heading. If using items only the number will be a whole number for each item.
5. For each item you can add a reference (described under Adding Regulations to Audit Checklists)
You can add multiple regulations to one checklist item if required.
Items and headings can be removed by clicking on the dustbin (delete) icon. Headings and items can be moved by clicking on the six dots at the end of each row and dragging and dropping into its new position.
*Remember to click on the ‘Renumber’ button to reorder the number Check number
6. When all headings and items have been added click on the ‘Save’ button
Moving back to the overview page of the checklist that you have created you can view the created sections and the number of items contained within each section.
The other viewable areas are.
Checklists Attachments – Any attachments connected to or link to this checklist
Audits in progress – Displays details of any audits of the same type that are currently in progress
Past Audits – Displays details of any audits of this type that have been previously completed.
Your checklist has been created and is ready to use.
