Adding a Defect
It is important that when an individual uses a piece of equipment that it works, this means recording maintenance for that piece of equipment as well as logging any defects that might occur, once a defect is logged it’s important to then track it against its impact to the business and record its rectification. This can be all achieved within Centrik, additional configuration can also alert an individual in an organisation when a defect has been raised.
Adding a defect when part of a check
1. Navigate to the piece of specific equipment, either through the equipment landing page, or by using the equipment search function
2. Click on the ‘Add Check’ button
3. Carry out the list of checks, selecting from the dropdowns available. If an item fails the checklist due to a defect, select this from the available list and add a comment if necessary
4. Within the Defects section click on the ‘Add’ button
5. In the Defect section complete a title of the defect, and select a priority, and a severity of this defect to the business from the dropdowns available.
6. The Owner section will be populated by the person raising the defect
7. The Responsible Person section is the individual responsible for fixing this defect, there is a selectable option of this individual being an External Contact, in essence they don’t have access to your Centrik system, selecting this as YES will create areas for an External Contact Name and External Contact Email to be added. If the individual responsible is an internal contact, then typing in their name in the available field will allow you to select from the list.

7. Click on the ‘Save’ button
8. Navigate back to the check that’s being undertaken, you can see that the defect has been raised. If the responsible person is an internal contact it will be displayed, if they are an external contact it will not be shown.
There is a pop out box also displayed, clicking on this will show the details of the defect in a more granular way.
10. Decide if this defect will Fail the check being carried out, or if the check is OK to sign-off but requires repair at some stage.
This can be actioned within the Result area, click on the ‘Edit’ button and choose from the dropdowns in the Result field
11. Click on the ‘Save’ button
Navigating back to this specific piece of equipment will highlight any open defects, there is also the ability on this page to add a defect outside of a check
1. Clicking the ‘Add’ button within the Open Defects section will open a new defect page
2. Complete the defect in the same way as described previously
3. Navigating back to the equipment page will display the additional open defect.
You can view all defects by clicking on the ‘View All Defects’ button.
Alternatively clicking on the pop-out and scrolling through using the ‘Next’ and ‘Previous’ buttons.
Adding a defect - Tasking Style (part of a check)
1. Navigate to the piece of specific equipment, either through the equipment landing page, or by using the equipment search function
2. Click on the ‘Add Check’ button
3. Click on the ‘Save’ button
4. Carry out the list of checks, selecting from the dropdowns available. If an item fails the checklist due to a defect, click on the ‘Defect’ button
5. Complete the defect pop-up box with details of the defect and its Priority, and severity to the business
6. Click on the ‘Create’ button
You cannot assign a defect to an individual from this page, however this can be actioned after raising the defect.
7. Once complete, navigate back to the specific piece of equipment and click on the open defect just created
8. The Owner section will be populated by the person raising the defect

Adding a defect outside of a check for tasking style is the same method as for checklist style.
Defects Overview
All raised defects are housed within the Defects button on the equipment landing page, any equipment that has a defect is also annotated on that class of equipment.
Clicking on the ‘Defects’ button displays the defects summary page, this page is defaulted to show Open and deferred defects only, open defects first but this selection is changeable within the Defect status list, there is also the ability to filter by date, search using text, by Vessell/Equipment, and by Status.
You are also able to search by selecting a defects pop-out box and scrolling through defects using the ‘Next’ and ‘Previous’ buttons if required.
Rectifying a Defect
Any named individual responsible for rectifying a defect will be notified via email and if a Centrik user through their personal dashboard.
Clicking on the Email link or navigating through their open defects will direct them to the defect that they have been named against.
Delegate a defect
The individual can delegate the rectification of a defect by using the ‘Delegate’ button, the named individual is still the responsible owner, but they are able to list another individual as a delegated party.
1. Follow the link to the open defect, or open the defect from the equipment landing page
2. Click on the ‘Delegate’ button
3. In the available box Rectification delegated to enter the internal contact’s name and select from the list that’s generated
4. Click on the ‘Save’ button
Deferring and rectifying a defect
Within a created defect the responsible owner is able to defer a defect or resolve/rectify an open defect.
Both areas are available within the defect itself.
1. Follow the link to the open defect, or open the defect from the equipment landing page
The two areas of focus are the Deferral area and the Rectification sections
Deferring a defect
1. Click on the ‘Edit’ button
2. In the available Deferral box, enter in the details of who is deferring this defect, the date of deferral, the date that it is deferred to, and the justification for the deferral
3. Click on the ‘Save’ button
Navigating back to the defect summary screen highlights that this defect has been deferred, once again the pop-out box is available to show granular details regarding this defect.
The status of the defects button also changes
Rectifying a defect
Rectifying a defect is a similar process to deferring a defect.
1. Click on the ‘Edit’ button
2. In the available Rectification box, enter in the details of who is rectifying this defect, the date of rectification, and Rectification details.
3. Click on the ‘Save’ button
Navigating back to the defect summary screen this defect will not appear as it has been rectified. However, you can filter the summary page to show all defects.
Clicking on ‘Search’ button after selecting an option from the dropdown displays all defects, open, deferred, and closed.
Within each specific piece of equipment there is an Open Defects section, within this area is a ‘View all Defects’ button, this will display all the defects associated to this piece of equipment, and its status.
Retire Equipment
If a piece of equipment is surplus to requirements or if a raised defect means that a piece of equipment must be sent to a specialist for repair then it might be out of your asset list for a few weeks, or months. In this scenario there is a ‘Retire’ button at the bottom of each specific item of equipment, pressing this will remove it from your asset list, once retired this button changes to read ‘Restore’, clicking on this button will restore it to your asset library
