Checklist Creation
If a checklist doesn’t exist within your audit area you may have to create one.
1. Click on the ‘Create Checklist’ button.
2. Give your Checklist a number (this can be alphanumeric, numerical or have an alphabet value).
3. Then give the checklist a name.
4. Under Checklist Style there are five available options.
Classic – The ability to create checklists sections and create your own checklist / audit questions
Classic with evidence – The same as classic style but with an additional area to capture evidence
Regulation based – The ability to create a checklist audit based on a regulation, when linked to Regulations this will create your audit question directly from the Regulation
Regulation based with evidence – The same as regulation based but an additional area to capture evidence
Regulation based with Auditor Actions (IOSA style) – The ability to create a checklist that uses the IOSA style of questions and answers.
If enabled on your site checklists types can be linked to regulations as either references or as your audit question
Possible Answers
There are three answer types available for selection
Standard Audit Result - (Yes/No/In Part/NA)
Standard Audit Result - (Compliant/Not Compliant/Partially Compliant/Not Applicable)
IOSA Audit Result - This allows for the checklist to include an area for Effectiveness Criteria as well as Auditor Actions (available through from a selectable list, that includes
Identify, Observe, Assess, Trace, Check, Review, Crosscheck, Coordinate, Sample, Examine/Compare, Identify/Examine, Identify/Assess, Examine/Assess, Observe/Assess, Interview, Examine, Other Actions)
Audit Editability
Checklist is extendable (additional items may be added, but items copied are fixed)
Checklist is fixed
Checklist is editable
Description
An area to hold a description of your checklist
Schedule
Last Completed On – The date the last audit took place
Next Due – When the next audit is due
Override Next Due – The ability to override the next due date
There is also the ability to schedule this audit across 12 or 24 months (depending on the site configuration)
Form Sections
The ability to add in sections to your audit, these can be enabled by Centrik
Sections and Contents
This the area that will host your audit questions broken up into sections
1. Clicking on the ‘+Add New Section’ button will allow you to create an audit section. It will need a Section Number (numerical or alpha numerical) and a section name.
2. Adding items to the section is achieved by using the ’Add Heading’ button or the ‘Add Item’ button. A heading cannot hold a reference within it but an item can.
3. You are able to add references by using the ‘Add Reference’ button available within each item
4. Once all headings (optional) and items have been completed you are able to click the ‘Renumber’ button, this will place a numerical value for each heading or item
5. To add a reference, click on the ‘Add Reference’ button
Adding Regulations to Checklists
Regulations can be subscribed to and managed automatically though your Centrik site. These regulations when enabled can be linked to checklists to support audit questions or used as an audit question on its own.
Regulation items can be added when creating a checklist or retrospectively after a checklist has been created.
Selecting the checklist style of Classic and Classic (with evidence) will allow for a reference box to be available after the audit question.
1. Click on the ‘Add item’ button.
2. Click on the ‘Renumber’ button to create a checklist number.
3. Enter the audit question in the blank box.
4. Click on the ‘Add Reference’ button.
This will enable the reference area
5. Start typing the regulation (for example EASA ml.1)
6. Select from the dropdown list of regulations.
When a regulation has been matched through back of house configuration the Reference box will turn ‘green’ in colour, if the regulation is not recognised it can still be entered but the box turns ‘yellow’ in colour, there will be no additional information available for this regulation either.
You are able to add more than one regulation to each checklist item by clicking on the ‘Add Reference’ button.
Clicking on this button will display the regulation on the screen
Regulation Based & Regulation Based (with evidence)
Checklist Types of Regulation Based and Regulation Based (with evidence) are set up slightly differently. These checklists use the regulation as the checklist question.
1. Click on the ‘+Add New Section.’
2. Enter a section number and section name.
3. Click on the ‘Save’ button.
Under the contents section there is only one option available, Add item
4. Click on the ‘Add item’ button.
A new checklist box appears where the regulation reference can be entered, alongside boxes for the Desired outcome, the Suitability criteria and any Guidance notes for the auditor.
5. Enter in the regulation reference and select from the dropdown along with any other information required
6. Click on the ‘Save’ button
To remove an audit item.
1. Click on the ‘Edit’ button.
2. Within the pop out box click on the ‘X Delete’ button
3. Confirm this by clicking ‘OK’ on the warning box.
Checklist Attachments
The ability to add an attachment for this checklist.
Self Audits
Self-audit checklists are created in the same way that other checklists are, the main difference is that the audit is locked to the ‘Self Audit’ style of audit and there is no schedule associated with this audit style.
